ACKNOWLEDGEMENT OF RISK
I understand that participation in wrestling and related athletic training activities involves inherent risks, including but not limited to physical injury, illness, or permanent disability. I voluntarily choose to participate (or allow my child to participate) in Beat the Streets Detroit (BTSD) practices and activities conducted at U of D Jesuit High School, and I acknowledge and accept these risks.
RELEASE OF LIABILITY
In consideration for being allowed to participate in BTSD practices and activities at U of D Jesuit High School, I hereby release, waive, discharge, and hold harmless:
- Beat the Streets Detroit (BTSD)
- U of D Jesuit High School
- Metro Detroit Wrestling Association
- Their respective officers, employees, coaches, volunteers, agents, and representatives
from any and all claims, demands, or causes of action arising out of or related to participation in BTSD practices and activities, including injuries or losses occurring on school property or during BTSD-related activities, except in cases of gross negligence.
INSURANCE COVERAGE
I understand that all participants must be registered through Beat the Streets Detroit (BTSD) and that participation is contingent upon current registration. I acknowledge that registered participants are covered by BTSD and USA Wrestling insurance while participating in authorized BTSD activities.
MEDICAL AUTHORIZATION
I certify that the participant is physically able to participate in wrestling activities. In the event of an injury or emergency, I authorize BTSD staff and/or U of D Jesuit staff to obtain medical care as deemed necessary. I understand that BTSD and USA Wrestling insurance will apply in accordance with their coverage terms.
RULES & EXPECTATIONS
I agree that the participant will comply with all BTSD, and U of D Jesuit rules, policies, and codes of conduct while on campus. Failure to follow these expectations may result in removal from participation.
